CONSENT

I hereby confirm that I have given certain of my personal information to Coval for the purpose of using it for the accomplishment of the above purposes. I understand that this information will be retained until the fulfillment of these purposes and will then be destroyed when it is no longer necessary.

I understand why they need this information and how they got it. I also understand that they will be communicated to Coval employees and partners, deemed necessary to achieve the purposes mentioned, and that they will be kept in their systems securely until their destruction.
